Your first visit: what to bring
- Photo ID and insurance card
- All your medicines and supplements — or the bottles themselves
- Recent blood and urine test results
- Home blood pressure readings, if you check them
- A referral, if your insurance requires one
Insurance: We accept most insurance plans. Our staff can confirm yours when you call.
Please arrive early: 30 minutes before a new-patient visit and 15 minutes before a follow-up, so that no one after you is delayed.
For referring physicians
Please fax referrals with recent labs (BMP, urinalysis and urine albumin-to-creatinine ratio) and a medication list.
